Update: followed the sequence of suggested steps. Was able to play two laps of Nurburgring without losing the controller wheels.

are you using PS4 console or PS5 ?
or are you sure your DualShock 4 is the authenticate product ?

Oh, I didn't realise the other user wasn't the OP...
Looking back, I can see they said they are on a PS4 Slim and in the logs the controller has the ID for a DS4 rev 2

If it's 100% certain the DS4 was off when connecting it to the PC, then the only thing I can think of is a USB issue. There is an option to 'power save' on USB ports. Might be worth checking that is not set, although the OP did state they used a different PC, so bit of a long shot. When I am next on my rig I'll see what debug options are available to log the authentication pass-through.

some users are using USB hub, try to plug the DS4 to the mainboard directly instead. and I suspect some AMD chipset board is actually having some compatibility issues ( like HP reverb G2 VR )

Please make sure you power on the PS4 using the power button on the console, not from the DS4. Also, I used the PS button in G25 rather on the DS4. My DS4 controller is only connected to the PC and is not on for entire time.
